The image below shows the Narrative tab of the Case Note screen. Counselors can sign and lock the notes.
There are three signature lines to support supervision. Locked notes can not be changed, they can only be amended by using
the Addendum feature. A note can forwarded, usually to a supervisor, for review and additional signatures. Forwarded
notes show up on the recipient's Task List. You can also
attach another computer file to the case note. This is often
used to attach a PDF or a scanned image of some external document
for centers that are going paperless. The most powerful
feature on this screen is the Data Forms button which will let you
attached multiple additional forms to the note. Some of the
predefined Data Forms include client demographic information and the
CCAPS assessment measure. You can also create your own Data
Forms. |

| The Diagnosis tab of the Case Note screen displays the DSM™ evaluation attached to the case note. There is a separate screen (second image below) for entering the DSM™ evaluation that can be opened from this page. |


| The Data Form button opens a screen where you can add either pre-defined or user created forms to a case note. This is a very flexible and powerful feature. There as some images below of different types of data forms to give you some ideas on what you can created with this feature. Most of the pre-defined data forms can be be modified by you. A few of them are based on a standardized questions are a copyrighted instrument like the CCAPS and cannot be changed. If the data form is too long to fit on a single screen the form will scroll so you access the entire document. |
