Narrative

This screen shows the Narrative tab of the feature-rich Titanium Client Note.

Notice that there is a three-tiered hierachical signature area, and that notes can be forwared for supervision.

You can easily attach a document to a note, like a scanned consent form, referral letter, etc.

Diagnosis

After choosing the Diagnosis tab on the Client Note screen (above), users can complete the DSM™ Evaluation screen.

Notice that values can be copied from a previous diagnosis.

Data Form 1

The Data Form button on the Client Note screen (above) allows users to select and complete forms as part of a note.

The Data Form screen shown here is an example of a pre-defined form, the CCAPS 34 questionnaire.

You are free to use any of the pre-defined forms in Titanium, modify them to suit your needs, or your center can create its own custom forms.

Data Form 2

This is another example of a Data Form that could be completed as part of a Client Note.

This screen shows the pre-defined AUDIT alcohol screening questionnaire.

Data Form 3

This Threat Assessment form is an example of a custom Data Form and is not included with Titanium.
